A health history questionnaire can be downloaded here.  You can fill it out either by hand or on the computer; and you can either submit it by e-mail or bring it with you for your first visit.  Occasionally, due to system incompatibility, it may not be possible to download the form or the form may lose data during transmission.  Please contact us if you have any difficulties.
